YOUTH REGISTRATION / PARENTAL MEDICAL RELEASE WINTER FEST February 7 – February 9, 2020
REGISTRATION COST $25.00 PER PERSON PLUS RENTAL/LIFT TICKET
Student Covenant:
I agree to obey all instructions given during Winter-Fest. This means from the time of arrival until I am back in the care of my parents. I also agree to do my best to make this
event an enjoyable experience for everyone in attendance. I understand this can be done through eager participation in all activities, an enthusiastic spirit and a positive attitude.
Parent Covenant:
I hereby give full consent for my youth to participate in the Winter-Fest. In the event that my child requires any medical attention, I hereby give my full consent for the adult
advisors from my youth’s church to make decisions on my behalf as to the extent of that medical treatment. I understand that any and all medical expenses that are incurred for
my child are my responsibility to pay.
I further understand that every reasonable effort will be made to ensure the safety of the participants, but that skiing and snowboarding and the recreation center are high activity
sports with risk of injury. Should my youth be injured, I will not hold my youth’s church, the United Church of Big Rapids, Ferris State University or their representatives
responsible in any way. I’ve had the opportunity to read the descriptive literature on “WINTER-FEST CONTINUES”. My youth and I approve the use of photos taken
during the event being used for publicity by the United Church and the denominations.
I understand ski/snowboard helmets are strongly encouraged. I am aware of the risks if my child chooses not to rent or bring one to wear.
